- identification of the copyrighted work claimed to be infringed;
- identification of the material claimed to be infringing, and information reasonably sufficient to locate it (e.g. a URL);
- your name, address, telephone number and email address;
- a statement that you have a good-faith belief the use is not authorized by the copyright owner, its agent, or the law;
- a statement, under penalty of perjury, that the information is accurate and that you are authorized to act on the owner's behalf.

Our response
Upon receiving a valid notice, we will act expeditiously to remove or disable access to the material and, where appropriate, notify the responsible account holder. We may forward your notice (including your contact details) to that account holder.
Counter-notification
If you believe your material was removed by mistake or misidentification, you may submit a counter-notice to [email protected] containing your signature, identification of the removed material and its former location, a statement under penalty of perjury that you have a good-faith belief the removal was a mistake, and your consent to the jurisdiction of the applicable federal court. We may restore the material in 10–14 business days unless the original complainant files a court action.
Repeat infringers
In appropriate circumstances, Eveses will suspend or terminate the accounts of users who are determined to be repeat infringers.
